User responsibilities

If you are posting information on publicly accessible areas of this site, keep in mind that anyone can see it. The site is intended to encourage users to share information, and unless you explicitly state in a post that information is not to be used or copied, then it may be used or copied by any user. Do not post any patient or health care provider specific information (such as names, health numbers, or address).

Infringing content
If you believe that anything posted on this site offends your copyright or other intellectual property rights, is defamatory, or otherwise infringes your rights, you may email Salus Global at the email address at the bottom hereof. We will take actions we believe are appropriate in the circumstances.
Logon id’s and passwords
Your access to portions of the site requires a logon ID and password. It is your responsibility to protect that information, including without limitation, to use effective passwords that are not easily guessed or discoverable, and keep logon ID’s and passwords confidential. You may not use a logon ID that suggests you are someone you are not. You are responsible for any activity that occurs under your logon ID. You will advise Salus Global immediately if you discover any compromise of your passwords or suspect unauthorized use of the site using your identity.
Limitations
All promises made by Salus Global with respect to this website are contained in this agreement. No promises implied by law (whether described as warranties, conditions, representations, or otherwise, and whether relating to merchantability, fitness, non-infringement, or otherwise) apply.
You cannot collect any damages from Salus Global for any reason (whether under legal theories of contract, tort, negligence, strict liability, operation of law or otherwise) that are not direct damages or exceed the amount paid by you to Salus Global hereunder for any goods or services that the claim arises from.
Salus Global relies on these limitations when entering this agreement and setting its fees, if any. They are a fundamental and essential part of our arrangement, and apply even if this agreement has failed in its fundamental or essential purpose or been fundamentally breached.
Governing law
This site is governed by the laws of the Province of Ontario excluding any conflicts of law provisions and excluding the United Nations Convention on Contracts for the International Sale of Goods. Any legal action against Salus Global shall take place in the courts of the Province of Ontario. The parties attorn to the non-exclusive jurisdiction of the courts of the Province of Ontario.

Salus Global is not responsible for anything on the linked sites, including without limitation any content, links to other sites, any changes to those sites, or any policies those sites may have. Sales details
Salus Global tries to provide accurate information on the Site and Product, but errors may occur. Salus Global reserves the right to change the goods and services available and their prices without any notice or liability. Salus Global cannot guarantee goods or services advertised on the Site will be available when ordered or thereafter. Salus Global reserves the right at any time to reject, correct, cancel or terminate any order for any reason whatsoever. Refund policy
If for any reason you are not satisfied with your purchase, we will offer to review your decision with you and offer up to a full refund depending on the situation. To process this request, please email info@informedob.com proof of your purchase and a description of the reason you require a refund within 30 days of receiving your order.
Privacy
For this site in particular, the personal information collected includes your name, e-mail address and practice location, including the facility where you will be participating in the program. It will also include the results of any of the activities in which you have participated in relation to the program. In addition, it will include a log of your participation. This log of your participation will contain information about your participation, but not the results of your participation.
Test results, including any quiz and survey results, will be stored in a secure file together with your personal information and will not be released to any party without your personal written consent. Your individual results will be accessible to you only via a password specifically assigned to you. Your test results will be used to create aggregate test reports. These aggregate reports will be available to your facility. Aggregate data and reports may also be used by Salus Global for program administration, program analysis, assessment and marketing. Test results will be aggregated in such a manner that your personal information will not be disclosed. Reports summarizing your participation history including which activities in which you have participated will be accessible to the team at your facility and to any other authority responsible for administering the program for your facility. Activity reports may include your name.
Changing these terms
Salus Global may change these terms by posting notice on its site. These terms were last revised on January 8, 2016.
